Question: What are your eligibility requirements?

Answer: ElderHealth serves frail elders and adults with disabilities and chronic conditions. Some of our participants have diabetes or other chronic health conditions and benefit from skilled nursing services to monitor blood sugar levels and manage medications. Others are at risk for falls and benefit from rehabilitative exercises to improve strength and balance. Some participants have Alzheimer's or other dementia and are enrolled in our specialized programs for persons with memory loss.

Our Intake Coordinator can talk to you further to determine if your loved one is appropriate for adult day services.
